Medical Records: Each condition approved by legislation requires medical record documentation of the diagnosis. The type of documentation needed depends on the condition. If you have the necessary documentation, you can be certified through the standard process.
If you suspect you have one of these conditions but have not been diagnosed, I can see you in a separate medical appointment to evaluate your history, symptoms, and perform an examination along with ordering any x-ray or laboratory tests that are needed. This process could require several clinic sessions in order to establish your diagnosis and treatment plan, but often just takes one visit, depending on your condition. My fees for these sessions are my standard clinic fees & should be booked as a standard office visit on the schedule.
Refunds / Cancellation policies
If you are unable to attend your appointment, please cancel 24 hours in advance. No refunds will be issued for cancellation requests within 24 hours of the appointment time.
